CK Recreation Feedback

We are committed to offering high-quality Recreation programs in our community and your feedback is important to help us adapt registration, advertising, and financial assistance to the needs of our communities.

• The survey is voluntary. You can choose not to take part in the survey or to skip any question you would rather not answer. 
• All survey responses are anonymous unless participants choose to enter their email into the gift card draw.
• If you choose to enter your email into the gift card draw, emails are separated from responses prior to data analysis and communication of results. 
• Personal information will not be kept after gift card winners have collected their prize.
• The gift card draw is in no way sponsored, endorsed, administered by or associated with Facebook, Instagram, or the parent company Meta.

All responses will remain confidential and all personal information will not be shared with recreation program staff.
