Results will be reported as percentages and will be published in a manner that maintains anonymity.
Draw
At the end of the survey, participants will have the option to enter into a draw for one of four gift certificates to a local restaurant of your choosing. The information you share for the draw will not be linked to your survey answers. See official contest rules in the document section.
Eligibility
Survey respondents should be residents of Chatham-Kent, 16+ years of age, and represent the following immigration statuses.
The survey is offered in select languages which include English, French, Spanish, Arabic, Punjabi, Hindi and Ukrainian. The Hindi translation is available in paper copy only. If required in another language, contact cklip@chatham-kent.ca. Paper copies of this survey are available upon request.

Personal information, as defined by the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA), on this form is collected under the authority of MFIPPA, the Municipal Act, and all other relevant legislation.
